Ms. Porter gave King a mixture of whole milk, water and infant cereal. Ms. Porter woke King up when I arrived. She got damp cloth to wipe him down, changed his diaper and got him dressed for daycare. Ms. Porters interaction with King was limited this morning. She woke him up to get him dressed, but after she got him dressed he went right back to sleep. She woke him up again when it was time to leave. She gave him a bottle and we took him to school. Today Ms. Porter had to go down to welfare and apply for TRA. She changed her mind about applying for TRA, because she did not know what documents she needed. I told her she still needed to go and start the process. On Wednesday she was told by Ms. Santos, if she did not have a place to live by June 5th 2016 King will be removed from her custody. Even if she chooses to stay at Project Home after June 5th, if king is not in her custody, she cannot stay at Project Home it is a shelter for women and their children. …show more content…
The housing case manager took Ms. Porte’s statement about her housing situation. He wanted to know, why she was being discharged / referred out of Project Home. He asked her, how long was the program, what did she do for them to discharge her and if she had anywhere else to go. Ms. Porter answered his questions fully. He repeated many of his question. I told Ms. Porter he needed details. She became annoyed at which point I stepped in. The case worker had information we needed. I did not want to leave with knowing for she had other options. Once he was done taking Ms. Porter’s statement he gave her and TRA application and 30 days to bring in a letter of discharge. The reporting party (RP) stated there were a list of issues and concerns regarding the level 4 home. A resident named Tammy Sanchez was observed by Regional Center counselor staff member Constance Sifton standing on a street corner on Mooney Blvd., in front of MOR Furniture store unsupervised. The counselor from the Regional Center pulled over and spoke with the resident. The resident disclosed that she had been on the street for an extended period which was indicated by the resident being drenched in sweat and appearing dehydrated and shaking. The resident was initially with a staff person named Isabelle and her boyfriend.
Mrs. Jones is a 67 year old female who presented to the ED via LEO under IVC through DayMark Recovery Services. Per documentation Mrs. Jones has been non compliant with medications and has been experiencing symptoms of psychosis. At the time of the assessment Mrs. Jones appears calm and cooperative. She does present with tangential speech and vaguely answers questions. She recently was discharged from Novant health 1/3/17 and followed up with DayMark 1/11/17 with new changes in medications.

MSTT met with Lemuel to further review the discharge plan and examine how he feels about the discharge. Lemuel stated he wish there was more time but stated he 's ready to take on his responsiblities. Lemuel explained he 's ready to start working and completing his GED program. MSTT and Lemuel then went out into the community to continue the discuss at a local resturant. MSTT encourage Lemuel to continue on the positive path he 's on and to stay motivatied.

Officer Hill reported that Lidia Hernandez had been encouraged to report as Evelyn had been gone for 2 days.

Mrs. Navarro asked the CP if the CP can provide her with information about entering the shelter, at which the CP agreed to do so. Mentoring Program: The CP provided Mrs. Navarro with information about SCR and mentoring programs for Tyler to explore. Mrs. Navarro reported that she enrolled Tyler and Sidney for the PAL afterschool program, but Tyler refuses to attend-so, the child’s application is placed on hold, according to Mrs. Navarro. Tyler was questioned as to why he is not going to the after school program and based on the child response, he could not provide a proper answer.
Interview with Today, I went to 1H, an Inpatient Mental Health Unit, to talk to Ms. Apple about the PRRC Program. Ms. Apple was referred over to the PRRC by an APRN for supportive services. Upon my arrival, Ms. Apple was sitting in the music room looking at the newspaper. I informed her that I am a Social Work Intern from the PRRC and I wanted to talk to her about the program.
Once Hazel entered the program, she begin working with DHR and Secure Dwellings to develop a plan for her to get her child back. Hazel’s son is now back with her and his youngest brother, a family reunited because of the stability the program afford her. We are currently assisting Hazel and DHR with getting Hazel permanent housing and employment in Birmingham. If everything continues to progress, Hazel will be in her own place by or before the date of this board meeting.
