What outcomes will be analyzed and how they will be analyzed? Although the veterans are asked their full name and last four of their social security, the scanning provides the added comfort of providing the security or safety for everyone involved. The outcome of this research will show data that will be analyzed pre and post implementation of the BMCA system, which the approach is to show a significant change in the medication error rate. The outcome will be based on pre and post implementation of the barcode medication system by measuring the medication error rate. There will be 100 veteran patients that will be assessed prior to implementation of the BCMA, and 100 veterans post implementations. Observers that will analyze administration errors, presence or absence of an error in the dose of medication administered during the observation period. Describe the statistical analysis you will do relating this to the data collection tools you plan to use. The VA outpatient clinic …show more content…
Each veteran will be administered a new VA identification card that has “bar-lines” for scanning purposes. When a veteran’s medication is administered, the nurse scans the veteran’s card, in addition to scanning the medication that is to be administered. The veterans are asked their full name and last four of their social security number, however with the scanning of their cards, it provides the additional security when confirming their allergies, age, weight, and gender. The study will use a monitoring system that is used by the VA’s called Surveillance of adverse drug events (ADEs). This system is used for reporting observed and new ADEs at the VA (VA, 2014). VA ADERS allows individuals to report, track, and electronically submit serious adverse drug events to the FDA’s MedWatch system (VA, 2014). Nurses should be educated on new medication policy and procedure, as well as, protocols (Anderson & Townsend,
SOF – The single dominant start of frame bit marks the start of a message, and is used to synchronize the nodes on a bus after being idle. Identifier-The Standard CAN 11-bit identifier establishes the priority of the message. Lower the value of identifier, its priority will be higher. RTR – The single remote transmission request (RTR) bit is used by the sender to inform receivers of the frame type.
Standard 6: In military environment there are medical records of patients. Printing out the lab work or information of medications is done elsewhere, since there is not a printer close by my computer. These records are privileged papers because it is a baseline bloodwork for the program. Before handing the papers to the participant we have them tell me their full name and last four of their social security.
Statistics Assignment Seven This paper will use inferential statistics to test two different research questions. There will be two different types of statistical test that will be used. The first statistical test will be a Chi-Square for independence and the second test will be a Person r test. Both test will have the seven hypotheses testing steps explained, descriptive discussion of the variables, possible errors and critique of research methods and implications for research and nursing practice will be provided.
To the Government of Denmark, Given the escalating situation in Mali and prior efforts to support peacekeeping missions and recognizing the importance of the implementations of MINUSMA(2013), I have decided to send this letter. Additionally, I am concerned with the migration patterns in Europe and the influences of this on the political unrest within Denmark. I hope for future references that this recommendation with regard to Mali can inspire other decisions with regard to peacekeeping in the future. Denmark has always supported peace and nation building in Africa and will continue to do so. In 2011 Denmark has supported the French military intervention Serval as Denmark has previously recognized the importance of stabilizing this region.
Thank you for the your very informative discussion, I found the description on the seventh variable really interesting. How the progression of ones behavior can effect the reassurance of their significant other. Our behaviors are capable of effecting the way our significant others view the partner and how society views your behavior. This course really challenges some of my thoughts as well as help me developed a more in depth knowledge of the behavioral analysis field . The ability to prioritize target behaviors enables behavior analysts, to measure behaviors from a broad spectrum without encountering the behavior.
Outcome Measurements Measuring outcomes is a vital element proceeding evidence-based intervention measures because it ensures that the intended purpose of the interventions has been met. To certain that a program is effective requires evaluation, which is supported by the strength and reliability of the proposed intervention. The goal of implementing the intervention: The Beat diabetes and new leaf for AA programs aimed at reducing the incidence rate of diabetes as indicators of positive outcomes that the preventive measures are effective (Ricci-Cabello, et. al., 2013). The principal evaluation measures for AA at risk for T2DM with the new leaf intervention program will be an improvement of hemoglobin (Hg) A.1c measure (below 6.5%),
These systems identify if a patient is doctor shopping or if a doctor is mis-prescribing medications. They can also identify if a dispenser is selling drugs illegally or by the book. More research
In its report on patient safety, The Joint Commission (2016) mandates that a minimum of two patient identifiers should be used when caring for patients, including but not limited to the administration of medication, collecting blood samples, performing medical treatments and procedures, etc. Patient names, birthdates, telephone number, assigned identification numbers, or other person-specific identifiers can be used to identify individuals for who care or treatment is to be provided. The rationale behind this policy is twofold: it identifies the person who the procedure or treatment is intended, and it matches the procedure, treatment, and/or service to a specific individual (The Joint Commission, 2016). Per The Joint Commission (2016), outcomes for the use of two patient identifiers will result in less patient errors during the course of diagnosis and treatment.

Secondly, the way the resident receives his medications should consist of the CM stating what each of the medications are so the resident is aware what he is taking. By implementing this, the CM can do the final check of administering the medications. If the medications themselves could be barcoded and scanned in before popping the medication in the medication cup, this would help the CM double check the five rights as well. A bar-code electronic medical administration record (eMAR) technology associates several technologies into the medication administration process to provide the correct medication, dose, time, route, and patient. This technology will provide an additional check and implement safety (Poon et al., 2010).
Pharmacists are in a unique position to help. They have the ability and knowledge to implement programs as part of their daily practice to ensure that patients are adherent to their medications. As the medication experts, pharmacists should lead the way to improving medication adherence and providing optimal patient care. The provisions of this bill do not pose a mandate but where appropriate would require the proration of prescriptions, related cost sharing, and dispensing costs in order to conform the patient to one monthly refill that occurs on the same date each month.
Kaiser Permanente has been equipped since 2007 with Health Connect; which is the largest private electronic health record implementation in the world. This is a highly sophisticated electronic program that integrates inpatient, outpatient, and clinic medical records with appointments, registration, pharmacy, and billing for all kaiser members. In addition, this electronic program includes an entire medical library with a whole set of care support tools which are accessible to doctors, nursing staff and patients (Kaiser Permanente, n.d.). At kaiser permanente; nurses are expected to print out “the after-visit summary” (AVS), which contain the doctor recommendations for each patient that we see.
These letters provide additional medication warnings to doctors and other healthcare
Medication Errors in Healthcare The nursing profession entails many responsibilities that range from providing emotional support to administering medications that could result in death for those receiving care. Approximately 40% of a nurse's day consists of passing medication, a duty that sets their level of liability above many other healthcare professions (McCuistion, Vuljoin-DiMaggio, Winton, Yeager, & Kee, 2018). Despite today's advances in technology and nursing education, the frequency of medication errors is still staggering. To ensure that the benefits of nursing outweigh the risks, nurses look to the Quality and Safety Education for Nurses (QSEN) six core competencies for guidance.
This helps clinicians make better treatment decisions by providing more continuous data. PHRs have the potential to help analyze an individual’s health profile and identify health threats and improvement opportunities based on an analysis of drug interaction, current best medical practices, gaps in current medical care plans, and identification of medical errors. Patient illnesses can be tracked in conjunction with healthcare providers and early interventions can be promoted upon encountering deviation of health status. A personal health record not only allows you to share information with your care providers but also empowers you to manage your health between visits.
