Ms. Martin is a 14 year old female who presented to the ED after an altercation between her mother and she. She denies suicidal ideation, homicidal ideation, and symptoms of psychosis to nursing staff. At the time of the assessment Ms. Martin is calm and cooperative. She reports tonight her mother would not let her eat a pop-tart for dinner and she became anger and proceeded to hit her mother. Ms. Martin states she only gets angry at her mother and lashes out at her. Per documentation Ms. Martin also lashes out at her grandmother with verbal and physical aggression. Per documentation LEO was called out the home. This clinician contacted the officer who respondent to the call. Officer Stacker who informed this clinician, Ms. Martin reported having an altercation with her mother because she told her she could not eat the pop-tarts for dinner. He reports she stated to him, "I don 't want to live." Ms. Martin was confronted about this statement and she states, "It 's not like I want to kill myself, I just don 't want to have to deal with my mother anymore." Ms. Martin appears tearful during assessment. She expressed feeling of depression as feelings of hopelessness, tearfulness, Insomnia, sadness, and anger. Patient has agreed to follow up with outpatient services as recommended. The patient 's family mother has been contacted and made aware of plans concerning the patient. Spoke to Vivian Ortiz (DOB 6/20/75) who stated that her juvenile son (16 years old), Elijah Perez (DOB 7/3/99), was being destructive and threw things around the house when she asked him to turn off the oven after he used it. Ortiz stated Perez had been diagnosed with intermittent explosive disorder and he had not been taking his medication or to his therapy. Ortiz also mentioned that she opened the case with DYFS to evaluate him and the case was still opened. Ortiz stated that Perez threw some baseball cards, laundry bin, trashes and made mess inside of the refrigerator. We weren’t able to speak with Perez as he walked out of the house prior to our arrival.
Mrs. Jones is a 67 year old female who presented to the ED via LEO under IVC through DayMark Recovery Services. Per documentation Mrs. Jones has been non compliant with medications and has been experiencing symptoms of psychosis. At the time of the assessment Mrs. Jones appears calm and cooperative. She does present with tangential speech and vaguely answers questions. She recently was discharged from Novant health 1/3/17 and followed up with DayMark 1/11/17 with new changes in medications.
C: Mrs. Mack (MHS) reported Jaheim Brown display physical and verbal aggression. A: At 9:58 PM Ms. Smalls (on call case coordinator) was notified by Mrs. Mack that Jaheim started throwing things around the home, refusing to comply with requests and refuse to communicate with Mrs. Mack. Mrs. Mack called law enforcement after Jaheim broke the bedroom closet door and other personal property in the bedroom.
Focus: Samantha will respect authority figures. Ms. Smalls (MHP), Ms. Smith (MHS), and Samantha process several incidents of noncompliance and defiance in the home. Intervention: MHP, MHS and Samantha discuss several reported incidents of defiance, refusal to comply and aggressive communication towards MHS. MHP educated Samantha of several possible outcomes because of her choices to disrespect MHS.
Extent of Maltreatment: The household consist of Shaquana L. Wright 37, (maltreating caregiver/mother), Demecia Harris 4, (victim), and Annie Howard (Grandmother). Shaquana Wright, 37(maltreating caregiver), mother of Demecia does not reside in the home Annie Howard (maternal grandmother/caregiver) but frequently visits the home of to care for the child. The maltreatment addressed in this case is Substance Misuse and Environmental Hazards. The child in the home is functioning normally. Child is not exhibiting hunger issues or looking unkempt.

Limitations recognised throughout the SDM process were related to risk of further deterioration in the Consumer’s mental state. As the Consumer was slowly taken off his medications, in a safe clinical manner, his presentation deteriorated. The Consumer’s sleep pattern worsened due to the elevation in his mood, there was a noted increase in impulsivity and poor boundaries with others on the inpatient unit, leading to the Consumer becoming vulnerable. There was a prominent increase in erratic and aggressive towards others, leading to the assault of a staff member on the inpatient unit and subsequently required the use of restrictive interventions. The decline in mental state resulted in the Consumer’s father, case manager and treating team coming together for a family meeting with the Consumer present in which the previous medications the Consumer had been previously prescribed were recommenced in an attempt to re-stabilise his presentation, unfortunately this was a substituted decision made by the consumer’s father and treating tream.
Callie is a 15 year old girl who ran track while in school. She experienced a tragic situation that drove herself to slit her wrist. Her mother sent her away to a rehab called Sea Pines hoping that she would get better. At rehab she has a therapist and 2 group sessions a day. It is filled with those who deal with food issues, anorexia, drugs, and behavioral issues.
The client is a twenty one-year old African American male, unwed and does not have any children and currently resides with his mother was referred to Evolutions Treatment center to address his Substance Abuse Disorder. Fred was born to an unwed couple in Long Island, New York and relocated to Wellington, Florida when he was two-years old due to his mother work schedule. He reported being primarily raised by his maternal grandparents until seventeen-years old, which he later went to live with his mother in Sunrise, Florida. The client described his relationship with his grandparents as "okay" and noted is grandparents, specifically his grandmother was overprotective of him. He noted that he was not allowed to leave the home without an adult supervision.
The reporting party (RP) stated while performing an intake of day care child Trevor Smith on 7/11/16 he disclosed his day care provider 's mother (name unknown) hits him on the head. According to the RP the mother would hit Trevor with an open head various places on his head per drawing completed by him. Trevor disclosed that the mother would hit him for being bad, however he could not remember the specifics. Trevor disclosed the latest incident occurred a few weeks ago; however the RP stated he was having difficulties remembering. The frequency of the alleged abuse is unknown.

Presenting Problem: Carter is currently admitted to the Devereux Intensive Resiidential Treatment Center as of 12/1/2015. Hx of being aggressive with family members. Admission occurred as a result of texting homicidal ideations regarding his mother, in which he threatened to cut her throat and or the brake lies on her car. He reports experiencing auditory and visual hallucinations sine the age of 12, records reports the hallucinations began at age 10. He denies feeling depressed but had symptoms of depression in the past.
" The patient reports that he is still struggling with the lost of his mother a year ago and a recent break up with his girlfriend of 42 years; which the break-up was about 4-5 months ago. Also, the patient reports that his truck was stolen from him by a individual he was about to give a ride. The patient reports in this incident the individual stole and blew-up his car. The patient has minimal eye contact during the assessment.
The patient is a 53 year old male who presented to the ED via EMS intoxicated and reporting suicidal thoughts. The patient denies homicidal ideations and symptoms of psychosis. The patient endorses depressive symptoms including: tearfulness, isolation, and insomnia. During the time of the assessment the patient is awake, alert, cooperative, and clam. the patient reports that he had been drinking to 2 pints of alcohol earlier during the day.
The reporting party (RP) stated adoptive child Sarah Middleton was observed with a re/pink swollen left hand. According to the RP Sarah complained of pain to the back of her left hand when pressure is applied. The child was given an ice pack for her hand. The RP stated the child disclosed that on 10/27/15 her adoptive mother Dianne Middleton was upset and hit the child with a "back-scratcher" on the back of her hand. The child did no mention the injury to her mother for fear of getting hit again.
